Inclusion of Name in Electoral Rolls
Eligibility
Citizen of India
Normally residing in the area falling within the Assembly Constituency
Public who have attained 18 years of age and above.
To whom to apply
Those eligible persons whose names are not included in the electoral roll of that constituency to apply in Form 6 to the Electoral Registration Officer.
Those in Chennai city apply to the Zonal Commissioners of the Chennai Corporation.
Those in Districts apply to the Revenue Divisional Officers.
Those in Corporation areas in the Districts apply to the Commissioners of Corporations.
Those in municipal areas in the Districts apply to the Municipal Commissioners.

When to apply:
The Election Commission announces schedule for annual revision normally in the month of November. Please utilise this opportunity to become a voter. Special Campaigns are also conducted on Second Saturdays and Sundays and fourth Saturdays and Sundays during this month at polling station locations, that is in school buildings nearest to your residence, to ease out the trouble of public in going to the office of the Electoral Registration Officers. Application Form 6 can be obtained from the same place free of cost and given back in the same place after filling up the forms. All the columns in the form must be filled up.
Document to be produced:
It is essential to produce residence proof to ensure that you are living in that constituency. Ration card, Bank passbook, Driving license, Identity cards given by employer or any one of the 16 documents prescribed by the Election Commission for voting can be used for this purpose.
Does inclusion necessary whenever the residence is shifted?
Yes. Unless your name is included in the Electoral Roll of that Constituency you cannot exercise your vote.

Furnishing wrong information or false information in the form6 is a punishable offence in the under section 31 of the Representative of People鈥檚 act 1950 .
