Make sure you let them no it's missing.

You will need to go to the local SS office along with your birth certificate and picture ID. It takes anywhere from 2 - 6 weeks to get a replacement card, I had to do that as my wallet was stolen twice, so I lost my original card as well as the backup. If you're talking about your Social Security card, you should find a social security office in your yellow pages and call them to verify exactly what you need to do, or go to http://www.ssa.gov and look for more info. Regardless, I'd recommend getting a new ss# given that you lost your old card. Better safe than sorry when it comes to identity theft.

To replace a lost Social Security card:
* Complete an Application For A Social Security Card (Form SS-5); and
* Show us documents proving your identity.
* Show us documents proving your U.S. citizenship if our records do not already contain that information.
* Show us documents proving your current, lawful, work-authorized status if you are not a U.S. citizen.
In most cases, you can mail or take your application and original documents to your local Social Security office. If you live in the New York City metropolitan area, Las Vegas or Phoenix you may need to apply in person at your local Social Security Card Center.
All documents must be either originals or copies certified by the issuing agency. We cannot accept photocopies of notarized copies of documents.
